Note that, since GWAS are currently underpowered to detect ratings in medical practice because they align more closely variance explained by the PRS is usually lower than individual in an independent genotyped sample; PRS are estimated as the sum of risk alleles weighted by their respective independently estimated effect sizes Overall, due the low missingness Essay on the power of money imputation should have little influence on the.

Depressive symptoms were measured using an 8-item Center for Epidemiological Studies Depression subscale and operationalised as both a dichotomous and a continuous variable. Instead, their results were compatible with an additive model.

Most recently, Mullins et al. Significant interactions were found between PRS and childhood trauma but, contrary to Peyrot et al. In summary, these studies do not present consistent results.

Given the higher lifetime risk of MDD in women 21 , we also tested the hypothesis in sexes separately. Materials and Methods Phenotypic data were collected as part of a general Health and Lifestyle questionnaire HLQ mailed to adult twins enrolled in the Australian Twin Registry between and 22 — The content and details of data collection have been previously described 22 — Data used in this analysis were collected in 3 waves.

This study was approved by the Queensland Institute of Medical Research Human Research Ethics Committee and the storage of the data follows national regulations regarding personal data protection.

All of the participants provided informed consent. The factor structure of the scale has been reported previously in the younger dataset 22 and the score has been used in several publications 9 , 22 , 23 , 28 , For PSLE, participants were asked to report adverse events divorce, marital separation, broken engagement or steady relationship, separation from other loved one or close friend, serious illness or injury, serious accident, burgled or robbed, laid off or sacked from job, other serious difficulties at work, major financial problems, legal troubles or involvement with police, living in unpleasant surroundings that happened in the last 12 months.

In addition, they were asked if they had had serious problems getting along with their close network spouse, someone living with you e. Several publications from our group have made use of these data 9 , 23 , 28 , 32 — It allows estimation of the probability of endorsing each option of each item as a function of the latent underlying trait known as Item Response Step Functions: IRSF , without any constraint on the shape of the fitted curve We used it to confirm the monotonicity of IRSF necessary to ensure the property of stochastic ordering on the sum score In such a model, the IRSF only differ in term of difficulty and discrimination 35 , Such IRT scores are maximum likelihood estimates of the latent trait and carry the same information as a sum score while presenting more normal distributions, thus reducing the influence of extreme values in later analyses.

The 1. Missingness not at random i. Thus, we chose to impute the missing observations using WinBUGS described in 34 , 41 using age, sex and the depression items as predictors. Overall, due the low missingness rate imputation should have little influence on the results.

Lifetime DSM-IV depression diagnoses were obtained in most of the cohort in two telephone interview follow-up studies using the clinical Semi-Structured Assessment for the Genetics of Alcoholism SSAGA 42 , 43 in — and — see Supplementary Figure 5 for a summary of the phenotypic data collection.

Details of data collection are described elsewhere 29 , 44 — Abstract Background Major depression is a widely used diagnostic category but there is increasing dissatisfaction with its performance. The diathesis-stress model is an alternative approach that does not require the sometimes arbitrary imposition of categories onto the spectrum of depressive morbidity.

However, application of this model has not been well explored and its consistency with available epidemiologic data is uncertain. Methods Simulation provides an opportunity to explore these issues. In this study, a simulation model based on an intuitive representation of diathesis-stress interaction was developed. Both diathesis and stress were represented using continuous distributions, without categorization.

A diagnostic threshold was then applied to the simulation output to create nominal categories and to explore their consistency with available information.

Results An apparently complex epidemiologic pattern emerged from the diathesis-stress interaction when thresholds were applied: incidence was time dependent, recurrence depended on the number of past episodes, baseline symptoms were associated with an increased risk of subsequent episodes and the remission rate declined with increasing episode duration.

Conclusions A diathesis-stress conceptualization coupled with application of a threshold-based diagnostic definition may explain several of the apparent complexities of major depression epidemiology. Some of these complexities may be artifacts of the nominal diagnostic approach.

These observations should encourage an empirical exploration of whether diathesis-stress interactions provide a more parsimonious framework for understanding depression than current approaches.

Keywords: Mood disorder, Major depressive disorder, Epidemiology, Mathematical models, Simulation Background Depressive symptoms can be measured using rating scales, which provide an assessment of symptom severity on an ordinal or continuous scale.

However, such ratings do not capture important aspects of the concept of a depressive disorder, as this is currently understood. Disorder definitions, and hence most available epidemiologic data, derive from nominal classifications e. Diagnostic categories typically play a larger role than symptom ratings in medical practice because they align more closely with clinical decision-making. Since publication of third edition of the Diagnostic and Statistical Manual of Mental Disorders DSM-III in [ 2 ], epidemiologic studies have embraced this nominal framework, as have subsequent editions of the manual [ 1 ] and the International Classification of Disease [ 3 ].

This nominal framework implicitly adopts a particular theoretical orientation towards the epidemiology: an incidence-prevalence-duration framework.

A central aspect of this way of thinking is that the clinical course is characterized by discrete and identifiable transitions between disease states. This framework leads to many familiar epidemiologic concepts e.

Categorization of an ordinal or continuous variable into nominal groups can create arbitrary distinctions and runs the risk of obscuring clinically meaningful features. Perhaps in recognition of this, the depression literature has seen the emergence of a complex terminology such as: partial remission [ 1 ], residual symptoms [ 5 ] and sub-syndromal or sub-threshold episodes [ 6 , 7 ].

Epidemiologically, episodes of the latter type may be difficult to clearly distinguish from non-pathological fluctuations in mood status and adjustment disorders [ 8 ]. These difficulties are also evident in the small literature of simulation studies of this condition. All such studies to date have represented the epidemiology from a nominal perspective, employing the general epidemiologic paradigm described above [ 9 - 12 ]. This result has subsequently been confirmed empirically [ 13 , 14 ].

However, the impact of these simulation studies has perhaps been diminished by the complexity of their modeling strategies, e.

In order to explore this, it is necessary to initially adopt a non-categorical diathesis, treating relevant variables as continuous ones, and then to apply a threshold-based definition to the data. Perhaps in recognition of this, the depression literature has seen the emergence of a complex terminology such as: diathesis remission [ 1 ], residual symptoms [ 5 a mean of zero and a user-assigned standard deviation 7 ]. Implicit in this theory is that there will be not only additive but multiplicative interactive effects on the liability scale 7. This result has subsequently been confirmed empirically [ 13 , 14 ]. Materials and Methods Phenotypic data were collected as part of a general Health and Lifestyle questionnaire HLQ mailed to adult twins enrolled in the Australian Twin Registry between and 22 — Overall, due the low missingness rate imputation should have little influence on the results. Initially developed to explain the origins of schizophrenia in the s 5 , 6 and adapted for the study of depression in the s 1 — 4 , this model states that stress may activate a diathesis or vulnerability, transforming the potential of predisposition into the actuality of psychopathology 7.

Materials and Methods Phenotypic data were collected as part of a general Health and Lifestyle questionnaire HLQ mailed to adult twins enrolled in the Australian Twin Registry between and 22 — The goal of this study was to explore whether a simple diathesis-stress model could reproduce some of the complex patterns typically seen in epidemiologic studies when a nominal diagnostic definition was superimposed on the continuous output. However, the impact of these simulation studies has perhaps been diminished by the complexity of their modeling strategies, e.

Consistent with this idea, adults having a history of childhood adversity show increased reactivity in stressful circumstances and may therefore be at higher risk of MD, see reviews by Taylor et al.

Introduction A popular explanation for the aetiology of depression is the diathesis-stress model 1 — 6. Such IRT scores are maximum likelihood estimates of the latent trait and carry the same information as a sum score while presenting more normal distributions, thus reducing the influence of extreme values in later analyses. In this study, a simulation model based on an intuitive representation of diathesis-stress interaction was developed.

Meztikinos

The values of each variable were updated in each day of the simulation as the agent moved across the environment, each day coming into contact with an area characterized by a specified stress level. Such IRT scores are maximum likelihood estimates of the latent trait and carry the same information as a sum score while presenting more normal distributions, thus reducing the influence of extreme values in later analyses. Note that, since GWAS are currently underpowered to detect all common genetic risk variants in complex traits, the variance explained by the PRS is usually lower than the twin heritability Some of these complexities may be artifacts of the nominal diagnostic approach. P-values as well as direction of effect are reported as previously.

The model proposes that there is a synergism between the diathesis and stress that yields an effect beyond their combined separate effects into depressive symptomatology and thus, the effects of stress on the depression risk are dependent on the diathesis. Depressive symptoms were measured using an 8-item Center for Epidemiological Studies Depression subscale and operationalised as both a dichotomous and a continuous variable.

However, he criticised the vague formulations for the predispositions and stressors that these theories propose. We used polygenic risk scores for major depressive disorder MDD calculated from the results of the most recent analysis from the Psychiatric Genomics Consortium as a direct measure of the vulnerability for depression in a sample of 5 individuals from 3 families. This framework leads to many familiar epidemiologic concepts e. The model used three variables to depict this process: stress activation, stress adaptation and stress burden.
